Card Access Request & Door Schedule Change Request

Please utilize the below form to request access for new areas. The request must be made by faculty/staff with authorization to grant access to the area requested. Requests that do not have the proper faculty authorization will not be processed.

  • Please do not send emails to Door Access. Utilize the above form only, due to the information needed to process the request. If the information provided does not match the record in the system (legal name listed) the request will not be processed.
  • For large numbers of students that need access, a file can be uploaded instead of manually entering the information. Please make sure the student/employee name matches the record and also provide the student ID number / employee number.
  • We will do our best to accommodate requests, but it takes time to change access hours for buildings and grant access to cardholders. Please make sure to submit requests at least 3 days in advance of the hours changing or access needed. Any requests that are not made at least 3 days in advance will be processed if they can be granted but not guaranteed to be completed.
