What is Form 1095-C?

Form 1095-C is an IRS form which shows information about your health insurance coverage for the tax year. This is a separate form from your W-2. This form provides proof of the health insurance coverage Furman University offered to you and your family during the 2025 tax year. This form is provided to you regardless of whether you elected to enroll in Furman’s health coverage or if you declined Furman’s Health coverage.

 What’s Changing?

Starting this year, your 1095-C form will be available online only. We will no longer automatically print and mail this form to everyone. If you need a printed copy, you have the ability to download and print from Workday or you can request one (see instructions below).

 When Will My 1095-C Form Be Available?

Your 2025 form is available now in Workday.

How to View Your 1095-C Form Online:

  1. Log in to Workday
  2. Click the Benefits and Pay App
  3. On the left side, click Benefits, then ACA Forms
  4. Find your 2025 1095-C Form (past years’ forms are also available here)
  5. Click View/Print
  6. You can now view, download, or print your form

 How to Request a Printed Copy: If you need a paper copy mailed to you:

  1. Send an email to: [email protected]
  2. Include this information:
    1. Your full name
    2. Your date of birth
    3. Your mailing address OR email address

You will receive your printed form within 30 days.
