This form must be submitted by 5 p.m. on October 2nd, 2009. The managers meeting for Flag Football, Team Handball, Spades and 9 Ball is October 6,2009, at 3:45 p.m. in the PAC, Room 116.

The meeting is mandatory and if you or a team member/organization member is not present at the start of the meeting, entrance to the meeting will be denied and the following penalty will be enforced:
Championship Team - loss of 25 entry points per team and 1 automatic loss

Recreational Team - loss of 25 entry points per team 1 automatic loss
