Preamble:
Medical Sociology and Medical Ethics are not your average collegiate course. There is a large fieldwork component attached to these courses. The reading load, while not particulary difficult, is extensive and essential. The student evaluations from previous classes show that your peers have commented on the challenging workload, and yet have enthusiastically stated the course has been one of the best they have taken. In order to assure that optimal learning takes place, members of the class must agree to the following course requirements and behavioral expectations.
Course requirements:
I understand that I will need to read all assignments ahead of time and be prepared to discuss them thoroughly in class. I realize that it will be necessary to complete some requirements before the first class meeting in January 2006.
I will turn in all papers on the due dates unless excused ahead of time by the professor for the course.
I will come to class everyday including any make-up Friday classes that it may be necessary to schedule due to weather. You are allowed no more than 3 absences. More than three absences will result in a lowering of your grade by 1/3 of a letter grade for each additional absence. For example, a B would be lowered to a B- for one additional absence. It would be lowered again to C+ for an additional absence.
If I am taking either of these courses pass/fail I promise to work as diligently as if I were taking the course for a grade.
Hospital Dress Code and Behavioral Requirements:
I agree to abide by all dress codes of the service on which I am rounding unless specified otherwise by the attending physician. Normally this will mean no jeans or T-shirts (men are typically expected to wear ties).
I will go to the hospital on time each day that I am assigned to a service. In the event of illness, I will contact my attending physician and my professor to let them know I will not be present. Medical excuses must be obtained through the Furman University Health Services.
I agree to secure all necessary shots, paper work, identification badge, immunization records and training sessions which may be required by the Greenville Hospital System prior to end of Fall Term 2004.
I understand that we are guests of the hospital and must behave appropriately at all times, so that this unique opportunity may continue in the future. I agree to behave in a mature and respectful manner including: showing up on time, dressing appropriately, acting attentive, not discussing private matters openly in public places, not removing hospital property (including but not limited to scrubs) without the permission of the attending physician, etc.
Confidentiality:
I agree as part of the requirements set out for these courses, to protect the privacy of the clients of Greenville Hospital System by withholding their identities and identifying features from all persons not connected with this class. Privacy extends beyond any identifying characteristics to revealing locations where clients are known to frequent, or any other information that could reasonably lead to the discovery of their identities. Even when discussing clinical cases in the context of class discussions, every effort will be made to protect patient confidentiality. Failure to comply with this agreement will result in a failing grade for the class.
Finally, I agree to follow all state and local laws, as well as the guidelines set out in the Helmsman and the rules of the Greenville Hospital System, throughout the duration of this course whether I am on Furman's campus or in the Greenville Hospital System. Failure to do so will result in a failing grade for the course. Please think very carefully before acting, your behavior may have an impact on the future success of this program.
I have read and understand this learning contract and agree to abide by the conditions set out therein. I realize that failure to do so may result in a grade reduction to be determined by the course instructor.
